Unlike the tech world, industry change in healthcare is often driven by less obvious, less sexy, less rapid changes. Move slow and tweak things. This chart from a recent Trilliant Health report captures the methodical shift of care from hospital to non-hospital settings. The most important thing to notice here is the types of cases being shifted. Not that long ago, the idea of replacing a joint or stenting a coronary artery in an ASC would have been heresy. Now it's becoming the norm. Glacially moving healthcare policy shifts are beginning to support this trend too. Lost a little in all the (justified) angst over reimbursement cuts, the CMS 2026 Final Rules put in motion the end of site-of-service distinction. If paired with site-neutral payments and the end of CON laws, this change could further shift healthcare dynamics away from large, complex, expensive, centralized health systems towards smaller, nimble, cost-effective outpatient facilities. Or, as I like to call them, micro-Centers of Excellence. Maybe the moratorium should be on building huge medical towers and sprawling campuses, not physician-owned hospitals. Some health systems seem to be getting the message and are developing ASC and outpatient strategies — mostly M&A and third-party facility development agreements. Those that aren't better hope the article is correct in assuming that the void of case migration will be filled by newer technologies and techniques. I'm not so sure. That ship, and the expensive math associated with it, may be leaving the harbor. If the current trend continues, we'll only see increasingly complex care shift to outpatient settings. We’re proud to congratulate Tigran Garabekyan, MD orthopedic surgeon at Commons Clinic, on his recent presentation at the 2025 ISHA Annual Scientific Meeting in Fukuoka, Japan. Dr. Garabekyan presented on the CU PAO—a minimally invasive, two-incision, interlocking periacetabular osteotomy (PAO) technique originally developed at the University of Colorado, where he contributed to the early research and outcomes of this approach. His talk explored how the CU PAO improves safety and recovery for patients with hip dysplasia—reducing the risk of nerve injury, enabling early weight-bearing, and offering smaller, cosmetic incisions compared to traditional techniques.
