Diabetes continues to steadily increase over the last two decades, with more than 38 million Americans living with the disease. We’re facing a critical health challenge that demands accessible, proactive care. The statistics are sobering. Diabetes accounts for 25% of health care spending in the U.S., and between 48% and 64% of lifetime medical costs stem from complications of unmanaged blood sugar levels. But here's what gives me hope: most type 2 diabetes cases can be prevented, and for those already diagnosed, personalized support can make a difference. At CVS Health, we're committed to meeting patients where they are, and at Oak Street Health, part of CVS Health, our value-based care model can have a real impact on our patients. Those who completed three or more nurse-led diabetes management visits over a four-month period had a 20% higher rate of HbA1C control.
