About us
Person Centered Services provides tailored care coordination services to individuals with Intellectual and/or Developmental Disabilities (I/DD). When New York State recognized the need for an improved, comprehensive, and conflict-free care management model for individuals with intellectual and developmental disabilities, Person Centered Services answered the call. We are a Care Coordination Organization (CCO) that was formed by 31 existing developmental disability service providers and agencies, and today we employ over 600 highly experienced care coordinators with offices in each of the 18 counties we serve. Care Coordination connects the dots between all aspects of an individual’s care. A Life Plan, a holistic plan tailored to the individual is created to encompass every aspect of an individual’s care, including disability-related supports and other services like medical, dental, wellness, behavioral health, job assistance, housing, safety, transportation and more. Plans are constantly monitored and modified as an individual's goals, needs and life circumstances change. At Person Centered Services we're all about serving people. Our mission is to connect individuals to the care, support and opportunities that maximize their quality of life so they can reach their potential.

Our team members are celebrating another great year of success! A pod based in West Seneca under Care Coordination Director Diane Kauffman held an end-of-the-year celebration. The event featured a lunch provided by Don Juan’s, as well as a group art session at "Painting with a Twist." There were 32 in attendance who created paintings during a two-hour session. "It was great to be able to step away from work, hear the laughter and see everyone relaxed, having conversations not work-related, and enjoying each other," said Diane Kauffman.
