For Moda Health, ReferWell operationalized outreach using our high-touch care navigation model to connect members to care, send follow-up reminders via their preferred channels, and check in with them post-visit to reinforce satisfaction and address any concerns. Reaching over 2,000 members in total, calls were intentionally structured to go beyond a transactional interaction to deliver comprehensive and humanized member experience that aligned with both clinical and experiential goals.     Beginning on January 1, the CMS Transforming Episode Accountability Model (TEAM) policy will reshape how care is delivered and coordinated to support #Medicare patients throughout their surgical journey. Up to two-thirds of participating #hospitals may lose revenue under TEAM, with 714 hospitals mandated to participate. That means, for the hospitals managing ~1,000 annual TEAM-eligible episodes, improvements in follow-up completion and care routing could translate into $7.5-20M+ in protected revenue annually.
