Think about it — 𝙋𝙝𝙖𝙧𝙢𝙖 𝙩𝙚𝙖𝙢𝙨 designing patient support programs, 𝙈𝙚𝙙𝙏𝙚𝙘𝙝 𝙘𝙤𝙢𝙥𝙖𝙣𝙞𝙚𝙨 building remote monitoring tools, 𝙋𝙪𝙗𝙡𝙞𝙘 𝙃𝙚𝙖𝙡𝙩𝙝 𝙖𝙜𝙚𝙣𝙘𝙞𝙚𝙨 sharing test results... They all face the same challenge: How do you make people feel that their time matters? And this is no longer an optional extra. 𝙄𝙩’𝙨 𝙖 𝙘𝙤𝙧𝙚 𝙥𝙚𝙧𝙛𝙤𝙧𝙢𝙖𝙣𝙘𝙚 𝙞𝙣𝙙𝙞𝙘𝙖𝙩𝙤𝙧. Because when people feel heard, understood, and valued, they’re more likely to stick with your care plan. They follow through. They stay engaged. They tell others. ECM vs. CCBHC — what busy clinic teams actually need to know Both aim at whole-person care and less fragmentation, but they’re different: What they are ECM (Enhanced Care Management): a benefit focused on organizing care across medical, behavioral, and social services for high-complexity members. CCBHC (Certified Community Behavioral Health Clinic): a clinic model delivering comprehensive mental health/SUD services with same-day access, coordination, and quality reporting. Who they serve ECM: people with multiple chronic conditions, homelessness, SMI, justice-involved, etc. CCBHC: anyone needing timely behavioral health care, including crisis. What success looks like ECM: fewer avoidable ED visits, completed care plans, warm handoffs, referral follow-through. CCBHC: faster access, evidence-based care, crisis stabilization, improved PHQ-9/AUDIT-C, coordinated transitions. Data you actually need (keep it small & reliable): ECM: panel & eligibility flags • assigned care manager • care plan dates/goals • referrals (sent/scheduled/completed) • touchpoints with timestamps • 7-day TOC after ED/IP. CCBHC: time to first contact/visit • screening scores (PHQ-9, GAD-7, SBIRT) • EBPs delivered (Y/N, date) • crisis encounters & resolution • coordination events • outcomes over time. Common pitfalls → quick fixes Free-text hides actions → add quick-click structured outreach fields. Referrals live in spreadsheets → move to EHR orders or a simple in-EHR tracker. Screenings done but not scored/trended → store discrete scores and graph them. No owner for data quality → assign a program “data steward” and do a 15-minute weekly check.
