About us

Your Community Health provides free and affordable medical, dental and allied health services and support programs through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services. Our services include: - Doctors – bulk billing - Dentists - Counselling - Diabetes Education - Nutrition and Dietetics - Physiotherapists - Podiatrists - Speech Pathologists - Social Support Groups - Men's Shed - Stop Smoking Support - Exercise Groups and Classes - Aboriginal health - Refugee health - Trans and gender diverse health - NDIS - My Aged Care We take pride in delivering a social model of health that meets your individual needs. We provide priority access to people who may face barriers accessing traditional health services. We understand the importance of addressing social, cultural, environmental and economic inequalities through our work. We have three health centres in Darebin and we also provide services in your home or in other community settings across Melbourne's north, such as leisure centres and senior citizen's centres. We are a not-for-profit company limited by guarantee registered as a Community Health Service under Section 48 of the Victorian Health Services Act.

    We are honoured to have won the ‘Creating a Sustainable and Climate Resilient Health System’ award at last night's Victorian Public Healthcare Awards! Our Climate Action work brings together staff, clients and community members to develop effective, practical solutions including fleet electrification, rooftop solar and better waste management, to improve our sustainability. These actions have cut our emissions by 33% and driven meaningful change across the organisation. Thanks to our team for embracing and championing climate action at Your Community Health. This recognition celebrates our shared commitment to creating a healthier, more sustainable Victoria. #VPHAwards

  • Meet Sandaleen! One of the passionate volunteers featured in our Annual Report. After moving to Darebin, she wanted to give back and feel more connected to her new community. Joining our Community Advisory Committee gave her the chance to do just that. From day one, Sandaleen has felt valued and inspired by the people around her.   • Today we mark World AIDS Day, an international day first held in 1988 to raise awareness about HIV and AIDS, remember those who have died from AIDS-related illnesses, and show support for people living with HIV. In Australia, an estimated 30,000 people are living with HIV, and worldwide, 39.9 million people are living with the virus, with more than 42.3 million lives lost to AIDS-related illness. World AIDS Day is a chance to raise awareness, eliminate stigma, and promote prevention, treatment, and care.     Very honoured to present alongside the wonderful Vanessa Brown (She/Her) yesterday at the ACON Pride in Practice Conference, sharing our Being Valued project from Your Community Health, TGV | Transgender Victoria and WorkSafe Victoria. Sadly, 1 in 2 LGBTQIA+ Victorians have experienced gendered violence at work in the last 12 months, with many still feeling unsafe to be out at work. From 1 December, workplaces will be required to address psychological violence under OHS law - meaning employers must actively prevent gendered violence, including purposeful misgendering, deadnaming and other harmful behaviours impacting LGBTQIA+ workers. The new Being Valued Toolkit offers practical steps, case studies and tools to help organisations identify risks, take preventative action and respond safely. Like so many of us, I’ve experienced my fair share of gendered violence throughout my career - including a very public online attack from people who were unhappy that I was working to amplify the voices of LGBTQIA+ young people. That moment unleashed psychological violence, malicious rumours, bullying, defamation, verbal abuse, doxxing and death threats. So I know intimately how deeply this type of violence can impact a person’s life. No LGBTQIA+ worker should experience harm doing something that should be safe for everyone - going to work. So if you’re a leader, HR practitioner, people-manager or ally of any variety, wanting to create safer, more inclusive workplaces and aren’t sure where to begin, the Toolkit is a great place to kickstart your learning journey. 🩷 You can download the new, FREE Being Valued Toolkit today 🩷

    What a thrill it was to attend and present at the National LGBTIQ+ Inclusion Conference - Pride in Practice! It was an honor to present with TGV | Transgender Victoria and introduce the Being Valued Toolkit, a collaborative effort from our amazing lived experience project team that sets a new standard for creating safer workplaces for LGBTIQA+ people. The enthusiastic response from convenors and thoughtful questions from fellow attendees showed a genuine commitment to implementing inclusive practices across all sectors. A huge thank you to ACON for having us and to WorkSafe Victoria for being such incredible ambassadors and supporters of this project! Together we are making a difference! #PrideInPractice #LGBTIQInclusion #BeingValued #AdvancingLGBTIQAInclusion
